Immunization Requirements
Colorado law requires students attending public schools to be immunized against certain vaccine-preventable diseases.
Students enrolled K thru 5th grade are required to have
- 5-6 doses of DTaP (Diphtheria, Tetanus, and Pertussis)
- 4 doses of the Polio vaccine
- 2 MMR (Measles, Mumps, Rubella)
- 2 doses of Varicella (Chicken Pox vaccine)
- 3 doses of Hepatitis B vaccine
All students entering the 6th grade are required to have
- 1 dose of Tdap (Tetanus, Diphtheria, and Pertussis)

Medical Exemption
A medical exemption is available for students who have a medical condition that prevents them from receiving one or more school-required vaccines.You are responsible for providing the signed Certificate of Medical Exemption to each school or child care you/your child attends.
A Certificate of Medical Exemption requires the signature of one of the following health care providers, licensed to practice in any state or territory of the United States:
- Advanced practice nurse (APN)
- Physician (Medical Doctor [MD] or Doctor of Osteopathic Medicine [DO])
- Physician assistant (PA)
A Certificate of Medical Exemption needs to be submitted only once, unless there is a change in the student’s school or personal information. Please visit the CDPHE website for more information.
Non Medical Exemption
A nonmedical exemption is an exemption to one or more school-required vaccines based upon a religious belief whose teachings are opposed to immunizations or a personal belief that is opposed to immunizations. This process may be completed by parents/guardians, emancipated students, or students 18 years or older.
There are two ways to submit a nonmedical exemption.
- Submit the Certificate of non-medical exemption WITH a signature from an immunizing provider in Colorado who is a medical doctor, doctor of osteopathic medicine, advanced practice nurse, delegated physician’s assistant, or pharmacist OR
- Submit the Certificate of nonmedical exemption received upon the completion of CDPHE’s Online Immunization Education Module which can be found on the CDPHE website.
Parents of students in grades K-12 claiming a non-medical exemption must submit one annually. Nonmedical exemptions expire June 30th each year. If you submit a Certificate of nonmedical exemption on or before June 30th, it will not be valid for the upcoming school year unless you submit the exemption during early registration.
